A typical hair transplant surgery lasts 4 to 6 hours, while total time spent at the clinic is usually 4½ to 6½ hours. Procedure length depends on whether FUT or FUE is performed, the number of grafts placed, and overall case complexity.
The answer depends on which portion of the surgical day is being measured.
Are we referring to:
- The operative time itself?
- The total time spent at the clinic?
These are two distinct measurements, and understanding that distinction is important.
Step 1: The Morning Preparation
At Armani Medical, surgery does not begin the moment the first graft is placed.
We dedicate approximately 30 minutes or more prior to beginning the operative portion to finalize surgical markings, confirm the treatment plan, administer pre-operative medications, and address any remaining questions before graft placement begins.
- Surgical consent is reviewed
- Pre-operative medications are administered (typically an anti-anxiety medication and prophylactic antibiotic)
- The surgical plan is discussed
- If the hairline is involved, it is designed using Dr. Armani’s ARTS™ (Artistically Redesigned Transplant Surgery) methodology
- The proposed hairline is reviewed collaboratively with the patient
- Pre-operative photographs are taken
About the ARTS™ Hairline Design
Dr. Armani’s ARTS™ methodology is a structured, artistic approach to hairline design that considers facial proportions, age, bone structure, ethnic characteristics, and projected hair loss progression. Hairline design is not simply a curved line drawn on the forehead, but a carefully crafted design tailored to complement the patient’s facial structure and mature naturally over time. Rather than attempting to recreate a prior hairline exactly as it once was, ARTS™ focuses on creating a result that is harmonious, realistic, and often better suited to the patient today than the original hairline may have been years ago.
Patients, along with any accompanying friend or family member, are then welcomed to our private patient lounge, where light snacks and a comfortable setting are provided.
Step 2: Entering the Procedure Room
Once in the surgical suite, final clinical preparations are completed before surgery begins.
- Recording patient weight (for medication dosing accuracy)
- Measurement of vital signs (blood pressure, heart rate, oxygen levels)
- Surgical site preparation, bandaging, or draping
- Any necessary hair trimming or shaving (depending on FUT vs. FUE technique)
Throughout the procedure, vital signs are continuously monitored to ensure patient stability.
Step 3: Actual Surgical Time
Surgical duration varies based on FUT vs. FUE, number of grafts, case complexity, hairline involvement, and density goals.
- Average Procedure Times:
- FUT: Approximately 4 to 4.5 hours
- FUE: Approximately 5 to 6 hours
FUE procedures typically take longer due to the prolonged and meticulous graft extraction phase.
At Armani Medical, Dr. Armani performs FUE using his trademarked PURE™ (Preserved, Undisturbed, Refined Extraction) methodology, which emphasizes controlled punch depth, minimized donor trauma, strategic graft spacing, and protection of surrounding follicular units. The goal of PURE™ is not speed, but preservation—safeguarding donor integrity while optimizing long-term graft viability.
Larger or more complex cases may require additional time.
A Note on Excessively Long Procedures
Dr. Armani has spoken with patients who had previously undergone hair transplant procedures elsewhere and was concerned to learn that some procedures reportedly lasted 10 to 12 hours or more in a single day.
Extended operative times may lead to increased patient fatigue, greater anesthetic exposure, increased team fatigue, and prolonged graft out-of-body time.
Even when grafts are stored in chilled preservation solution, follicles remain biologically stressed while outside the body. Excessively long procedures may therefore compromise both patient comfort and optimal graft survival.
At Armani Medical, procedures are deliberately structured and appropriately staffed to maintain efficiency—without rushing and without unnecessarily prolonging surgical time.
What Happens During the Procedure?
Throughout the day, patient comfort and safety remain our priority.
Most patients are completely comfortable once local anesthesia is administered, and discomfort is typically minimal.
- Watch Netflix or preferred streaming services
- View sporting events via satellite
- Listen to music
- Use their phone
- Watch programming on our large-screen television
After the Procedure
- The surgical area is dressed
- Post-operative instructions are reviewed verbally
- Printed instructions are provided
- Final questions are addressed
Patients receive a post-operative care kit containing essential recovery items. In most cases, Dr. Armani personally follows up the next morning to ensure recovery is progressing appropriately.
Driving After the Procedure
Although patients are generally alert at the conclusion of surgery, our policy is that patients do not drive themselves home due to residual medication effects, the length of the surgical day, and safety considerations.
Total Time in the Clinic
Approximately 4½ to 6½ hours at the clinic.
- FUT vs. FUE
- Number of grafts
- Case complexity
- Hairline involvement
- Individual patient-specific factors
These timeframes are estimates, not guarantees. Additional time may occasionally be required for anesthetic administration, patient comfort, technical adjustments, or other patient-dependent variables. Our priority is to complete the procedure properly, safely, and without rushing any critical step.
