Is Hair Transplantation Painful?

Hair transplantation, when performed by an experienced surgeon and team, is generally far less uncomfortable than most patients expect. Dr. Armani often explains that the procedure is typically less uncomfortable than a routine dental appointment, which many patients find reassuring once they understand how the process actually works.

To make this easier to understand, it helps to look separately at:

• Comfort during the procedure

• Comfort after the procedure

• Whether there are meaningful differences between FUT and FUE

During the procedure

It’s important to recognize that every patient has a different pain tolerance, and this naturally varies from person to person.

That said, several steps are taken to keep patients relaxed and comfortable throughout surgery. Most patients comment afterward that the experience was much easier and more comfortable than they had anticipated.

We typically provide a light oral sedative to help reduce stress and keep patients relaxed during the procedure.

For both FUT and FUE procedures, our approach to numbing the scalp is designed to keep discomfort to a minimum. At Armani Medical, we use extremely fine needles — approximately the diameter of a human hair — along with a gentle micro-vibration technique that helps reduce discomfort during the initial numbing injections. The numbing is performed slowly and gently, and we schedule only one procedure per day so that patients are never rushed.

As a result, most patients report little to no discomfort once the scalp is numb.

Many patients assume that FUT (strip harvesting) must be significantly more painful than FUE because it is more invasive. In practice, this is not the case for most patients.

FUT

Although removal of the donor strip during FUT harvesting typically takes only about ten minutes, the closure of the donor area is where meticulous attention becomes especially important.

Under Dr. Armani’s surgical technique, closure usually takes around twenty minutes and is performed very carefully. This is considerably longer than simpler closure methods that may take only a few minutes, but we believe the extra time plays an important role in patient comfort, healing, and the final appearance of the scar.

At Armani Medical, extremely fine sutures are placed with meticulous precision so that the closure appears almost machine-sewn. These sutures may cross the incision 50–80 times, distributing tension evenly along the entire incision rather than concentrating it in only a few points.

This approach allows the skin edges to heal naturally while preserving circulation to the margins, which supports comfort, healing, and a finer scar.

A carefully performed, low-tension closure using closely spaced sutures is one of the key factors in minimizing discomfort and optimizing healing after FUT.

FUE

FUE procedures generally take longer overall, which means more injections are required to numb both the donor and recipient areas. However, once the scalp is numb, patients usually remain very comfortable for the remainder of the procedure.

For both FUT and FUE, if any discomfort is felt, it typically occurs only during the first few minutes of numbing. After the scalp is anesthetized, patients commonly watch movies, listen to music, follow a game, or even fall asleep.

In terms of comfort during surgery, FUT and FUE are actually quite similar. Because the harvesting phase of FUT is often shorter than FUE extraction, some patients even find FUT more comfortable simply because that portion of the procedure is completed more quickly.

After the procedure

Post-procedure discomfort is usually mild for both FUT and FUE.

In our experience, the majority of patients take their prescribed pain medication only on the first night after surgery. By the following day, many do not require even over-the-counter medication, which often surprises them given what they expected beforehand.

It is true that FUT patients may experience slightly more discomfort for a few nights, primarily due to the sutured donor area. Because patients lie on the back of the scalp during sleep, this region can feel more sensitive initially. For this reason, we provide a supportive neck pillow designed to reduce pressure and improve comfort during the first few nights.

While individual experiences vary, most patients ultimately find the procedure far more comfortable than anticipated, and any discomfort during or after surgery is usually short-lived and very manageable.

FUE Mild discomfort mainly during the initial numbing injections. Once numb, most patients remain comfortable throughout surgery. Usually mild soreness for a few days, typically manageable with minimal medication.
FUT Very similar to FUE once the scalp is numb. Some patients even find FUT comfortable because the harvesting stage is shorter. Slightly more tightness or sensitivity in the donor area for a few nights due to sutures, but usually improves quickly.
