Scalp micro-pigmentation (SMP) is a cosmetic procedure similar to tattooing in which specialized ink pigments are deposited into the superficial layer of the scalp to simulate the appearance of hair follicles. Rather than creating actual hair growth, SMP produces the visual illusion of density or a closely shaved scalp. It is most commonly used in patients with thinning hair, visible scalp, or scarring from prior procedures, and in some cases for individuals who prefer the appearance of a shaved head.
SMP can be useful in selected patients. It may help reduce the contrast between hair and scalp in diffuse thinning, camouflage FUT scars, or create the appearance of fuller density when combined with existing hair. However, it is important to understand that SMP does not restore hair, does not stop hair loss, and does not replace transplantation when true hair restoration is desired.
The most dramatic cosmetic results are usually seen in patients who wear their hair very short or shaved, have darker hair, and maintain a relatively stable pattern of hair loss. SMP can also work well for camouflaging linear FUT scars or localized thinning when performed conservatively and artistically.
In general, the ideal candidate is someone with black or very dark hair, particularly when there is also a darker skin tone, since the reduced contrast between scalp and hair helps the pigment blend more naturally. Patients who are unlikely candidates include those with very light blonde, red, or gray hair, since pigment dots may appear darker than the surrounding hair over time. Patients with progressive or irregular hair loss may also find that SMP becomes less natural as untreated areas continue to thin. In addition, individuals who prefer longer hairstyles often find SMP less convincing, as the illusion is strongest at shorter hair lengths.
One important long-term consideration is aging and graying hair. As natural hair turns gray with age, the tattooed pigment does not change color, which can make the dark spots more noticeable over time. For this reason, SMP should always be planned with long-term aging in mind rather than just the patient’s current look.
As with any cosmetic or camouflaging procedure, SMP rarely looks completely natural under close inspection, and results depend heavily on technique, artistic judgment, and proper patient selection.
