When can I rinse or wash my hair after a hair transplant?

This is one of the most common questions patients ask after surgery, and there is no single universal answer. Every surgeon and clinic may have slightly different recommendations based on their experience and philosophy. The guidance below reflects Dr. Armani’s approach, developed over nearly 20 years of exclusive focus on hair transplantation and observing which patients achieved the safest healing and best long-term results.


The first 48 hours after the procedure

When patients leave after their transplant, they will have a bandage in place, which is covered by a disposable surgical cap. This allows patients to leave the clinic looking presentable — many patients jokingly say they look like a doctor after surgery without having gone to medical school.

We recommend that patients do not remove the cap, touch the bandage, or manipulate the scalp for approximately 48 hours after the procedure.

This serves several important purposes:

  • The bandage keeps the donor area clean and provides gentle pressure, helping minimize any chance of bleeding.
  • The cap provides cosmetic coverage.
  • The cap also protects the scalp from accidental touching or scratching during the early healing phase.

Before the bandage and cap are placed at the end of the procedure, the transplanted area is sprayed with HypoThermosol®, a specialized preservation solution used in advanced laboratory and transplant settings. This solution helps keep the follicles moist and provides metabolic support, including energy substrates such as ATP, during the earliest phase before the grafts establish their own blood supply. This is another reason why we emphasize minimizing any disturbance to the area during the first few days after surgery.


After 48 hours: two possible approaches


Option 1 — Gentle rinsing (no touching)

Patients may gently rinse the scalp using a cup or bowl of lukewarm water and a very mild shampoo such as Johnson’s Baby Shampoo. The water can be poured slowly over the scalp, followed by rinsing again with clean water using the same cup or bowl.

It is important that the transplanted area is not rubbed, touched, or manipulated in any way. This method avoids the pressure of a typical showerhead or handheld sprayer, which could dislodge grafts in the early days after transplantation.

Option 2 — Leave the transplanted area completely alone (preferred)

In Dr. Armani’s opinion, the safest option is to leave the transplanted area completely untouched during the first several days. This does not harm the grafts and is simply a more conservative approach to early healing.

Even gentle rinsing is not entirely risk-free:

  • Water temperature may vary and potentially affect the transplanted grafts.
  • Tap water contains chlorine and other potentially harmful substances.
  • Even mild shampoos contain chemical ingredients.

For patients who can tolerate the appearance of small scabs or dried blood spots, leaving the transplanted area alone provides the most protection for the newly placed grafts. Patients may gently clean the donor area if they wish, but even this is not strictly necessary.

Patients who choose not to rinse their scalp can still bathe normally. They may use a bathtub or shower to wash their body, simply keeping the head above the neck and out of the water to avoid wetting the transplanted area.

For patients who prefer not to rinse their scalp, our clinic provides shower caps so they can bathe comfortably and safely while keeping the transplanted area dry and protected during the first few days.


Care of the donor area

What is important is that beginning around 48 hours after surgery, patients apply a thin layer of Polysporin ointment or another triple-antibiotic ointment to the donor area (whether FUT or FUE). This helps support proper healing and protects the area while the skin closes.

Patients can shower their body normally right after the procedure.


The most important rule: nothing touches the grafts for 10 days

Either way — whether patients choose to gently rinse or not rinse their scalp during the first 10 days — the transplanted area must be left completely alone. Patients should not touch it, comb it, style it, blow dry it, towel dry it, apply products to it, or use topical medications such as minoxidil or topical finasteride on or near the transplanted area. The only contact allowed is gentle rinsing with a bowl and a mild shampoo such as Johnson’s Baby Shampoo.

This includes any prescription or over-the-counter topical treatments, including minoxidil, topical finasteride, or medicated lotions. Nothing should be applied to the transplanted area during the first 10 days.

For the first 10 days, the transplanted follicles must be left completely undisturbed.

During this period, patients should not:

  • Shampoo the transplanted area normally
  • Rub or touch the grafts
  • Scratch the transplanted area
  • Comb or brush the area
  • Blow dry the transplanted area or near the transplanted area
  • Towel-dry the transplanted area
  • Apply hair products
  • Use topical medications such as minoxidil or topical finasteride on or near the transplanted area
  • Allow hats or any objects to touch or press against the transplanted follicles
  • Place pressure on the grafts

The reason for this is that transplanted follicles require time to stabilize. Blood supply typically begins to establish within about four days, and studies show that grafts become firmly anchored by around seven days. Dr. Armani recommends waiting 10 days because this provides an additional safety margin and reduces the risk of graft displacement.


Dr. Armani’s “10-Day Rule”

Dr. Armani often explains what he calls the “10-day rule” to make postoperative instructions easier for patients to understand and follow. In general, when patients are unsure about what they can safely do after a hair transplant, the safest answer for most activities is about 10 days. This applies to washing, touching, styling, and using products on the transplanted area.

This recommendation is based on healing biology. Studies show that transplanted follicles typically begin re-establishing blood supply within about four days and become securely anchored in the scalp by around seven days. The 10-day rule simply adds an additional safety margin to ensure the grafts are fully protected during this critical period.

There are a few important exceptions. Returning to strenuous workouts after an FUT procedure may take longer, and hair coloring or chemical treatments are usually postponed for about four weeks after both FUE and FUT procedures.


Rinsing vs Not Rinsing During the First 10 Days

At that point:

  • Scabs can be gently washed away
  • Normal shampoo may be used
  • Hair can be combed and styled
  • Blow drying is allowed
  • Hair products may be used
  • Topical treatments such as minoxidil or topical finasteride can be resumed

At this point, patients can treat the transplanted area like the rest of their hair.

From this point forward, the transplanted follicles are secure and normal hair care can resume.
