Hair transplantation in African American patients presents unique technical and aesthetic considerations compared to other hair types.

One of the most important distinctions is that African American hair is typically more tightly curled externally. It is also important to recognize that the follicle itself beneath the skin is curved or curled, often resembling the shape of a question mark. This internal curvature increases the technical complexity of both FUT and FUE procedures. During harvesting, special care must be taken to account for this curvature, as failing to do so can increase the risk of follicular transection. This is particularly a challenge in FUE procedures, where smaller punches can create narrower grafts, which can be more susceptible to transection when the follicle is significantly curved beneath the skin. For this reason, grafts must be harvested in a way that preserves their integrity, often requiring slightly larger, more “chubby” grafts to minimize the risk of transection or damage to the follicle.

In addition, the angle and direction of hair placement require careful adjustment. In many African American patients, transplanted hairs are intentionally placed at slightly more upright angles compared to some other hair types. This is done because the natural curl of the hair increases the likelihood of ingrown hairs if they are placed too acutely or too flat against the scalp.

Hairline design is another important consideration. African American hairlines are often naturally straighter in appearance, and many patients prefer a more defined, “lined up” appearance with minimal irregularity. This aesthetic preference can differ from other hairline designs, which may incorporate more softness or subtle irregularity. As a result, detailed discussion between the physician and patient is essential to ensure that the designed hairline aligns with the patient’s natural features, ethnic characteristics, and long-term personal preferences.
