What Are Common Post–Hair Transplant Issues and How Do I Manage Them?

After a hair transplant, it is important to distinguish between expected post-operative changes and true surgical complications.

Two complications that should be extremely rare after a properly performed hair transplant include:

  • Post-operative infection
  • Significant or persistent bleeding

These events fall outside the expected course of recovery and are not considered routine post-operative findings.

If infection, abnormal bleeding, or other unexpected symptoms are suspected, patients should promptly contact their treating physician or surgical clinic for appropriate evaluation and medical management.

When surgery is performed in a properly controlled medical environment by an experienced physician and a well-trained surgical team, such complications are extremely uncommon.

By contrast, several temporary changes are normal parts of healing. These are predictable biological responses to precise surgical work performed on the scalp and typically resolve with time and proper care. Most patients resume normal daily activities quickly while these minor issues gradually improve.

Is mild swelling normal after a hair transplant?

Yes. Mild swelling after a hair transplant is a predictable and temporary response.

Swelling occurs as the body responds to anesthetic injections and graft placement. Surgical precision plays a critical role in minimizing this response.

Post-Operative Positioning:

  • Remain in a slightly reclined position

Ice and Medication:

  • An ice pack is provided (for the forehead only — never on grafts)
  • An anti-inflammatory medication is used as part of the post-operative protocol

If swelling occurs, it typically begins around 48 hours, peaks between days 2–4, and resolves within 5–7 days. Mild eyelid swelling or occasional bruising may occur and is self-limited.


Are scabs normal after a hair transplant?

Yes. Scabs are an expected and protective part of healing after both FUE and FUT procedures.

Nothing should be done to remove scabs during the first 10 days. After day 10, they loosen and fall off naturally. Picking or scrubbing before this time may risk graft displacement.


Is dry scalp common after a hair transplant?

Yes. Dry scalp often becomes noticeable after scabs have fallen off and the skin continues to recover.


IMPORTANT POST-OPERATIVE RULE:

Nothing should touch the transplanted follicles for a minimum of 10 days.

No oils, creams, products, or normal shampooing should be applied to the transplanted areas during this period.

After a full 10 days have passed and all scabs have naturally fallen off, gentle moisturization may begin if dryness is present.

Why does the scalp feel tight after a hair transplant?

Scalp tightness is common and temporary, more frequently associated with FUT procedures due to linear donor closure and temporary tissue tension.

Why am I itching after my hair transplant?

Itching occurs due to normal healing and no normal shampooing during the first 10 days.

Oral diphenhydramine (Benadryl) may be recommended and may cause drowsiness.


Is numbness normal after a hair transplant?

What is shock loss after a hair transplant?

Shedding does not mean graft failure. The follicle remains viable and will resume growth over time.


Is insomnia common after a hair transplant?

Temporary insomnia may occur due to head elevation, mild tightness, and temporary routine changes.

Short-term sleep medication may be provided for the first three nights. Beyond that, diphenhydramine may be used if appropriate. Long-term sleep medications are avoided.
