When can I work out after a hair transplant?

Many patients considering hair transplantation are physically active and go to the gym regularly, so this is a very common question. In general, exercising up until the day before the procedure is not a problem. After surgery, however, there are important differences in how soon patients can return to workouts, and these vary between FUT and FUE procedures.


The first week after surgery

During the first week after the transplant, patients can usually perform about 90% of their normal daily activities at home and return to typical office-type work if applicable. However, for both FUT and FUE procedures, we recommend avoiding the gym and any strenuous activity or exercise during this first week after surgery. During this early healing phase, it is best to avoid sweating, excessive heat, and any activity that significantly raises blood pressure. Keeping activity light and remaining in a comfortable temperature environment helps support proper healing.


Exercise after an FUT procedure

With an FUT procedure, a suture is placed in the donor area and typically remains in place for about 12 days. During this period, we want to protect the donor area so that it heals optimally and produces the best possible scar.

After about 10 days, patients who have had an FUT procedure can usually resume light activity and non-strenuous exercise, including walking or moderate cardio. However, we recommend avoiding very heavy lifting or strenuous workouts for approximately four weeks after the procedure.

This recommendation is not primarily for the transplanted hairs themselves, but rather for the healing of the donor scar. Beneath the donor area lies a very thin muscle called the occipitalis muscle, and when patients perform very heavy or strenuous exercises too early, that muscle can contract. This can increase tension on the healing incision and potentially lead to a wider or more visible scar.

A simple way to define what is too strenuous is something Dr. Armani often tells patients: if you have to hold your breath and grunt while doing the exercise, it is probably too heavy and should be postponed until later in the recovery period.


Exercise after an FUE procedure

With an FUE procedure, there is no linear incision or suture in the donor area. Because of this, once the small extraction sites have healed, patients can generally return to full activity sooner.

After about 10 days, most FUE patients can return to the gym and resume normal workouts, including cardio and weight training, as long as the scalp has healed comfortably and there are no individual concerns.


Individual variation

Every patient heals slightly differently, and activity recommendations can be adjusted based on the size of the procedure, the areas treated, and the patient’s overall health. For this reason, patients are always given individualized instructions during their consultation and postoperative visits.

Why the Difference? Very heavy or strenuous exercise can cause contraction of the occipital muscle beneath the donor area, increasing tension on the healing incision and potentially worsening the scar if done too early. Because there is no linear incision or suture in the donor area, there is no comparable tension on healing tissue, allowing patients to safely return to full activity — including heavy lifting — once the scalp has healed.
